As Chronic Kidney Disease (CKD) Specialist Nurses, our role is to provide you with the care, support, and education you need to manage your kidney health. Our aim is to empower you with the knowledge to make informed decisions about your treatment options, whether you’re in the early stages of CKD or living with advanced kidney disease.

Here’s how we can help you:

Pre-Dialysis Education

If you are in the stages of CKD where dialysis may be a possibility in the future (usually when your kidney function drops to below 20%), you will be referred to us by your kidney doctor so we can guide you through essential pre-dialysis education. We will help you understand the different types of dialysis treatments, the process of getting started, and when dialysis may be required.

 

We can talk about:

  • What your kidneys do in the body

  • Lifestyle modifications to maintain your current kidney function

  • Transplantation (if suitable)

  • Types of dialysis

  • Conservative management (supportive care approach rather than active treatment)

 

Together, we will discuss the options that best suit your lifestyle and goals for kidney care, ensuring you feel confident and prepared for the next steps in your treatment journey.

Anaemia Management

Anaemia is a common condition for people with CKD, and it can affect your energy levels and overall well-being. Not all people with CKD are anaemic, but if you are, your kidney doctor will let us know and as part of your kidney care team, we will monitor and manage anaemia by educating you on the causes, symptoms, and treatment options. Through regular checks and tailored care, we will support you in managing your anaemia and ensuring you feel your best throughout your kidney care journey.

Facilitating Patient Education Information Sessions

Education is key to understanding and managing CKD. We run regular patient education sessions, where we provide information about managing CKD, kidney-friendly diets and lifestyle changes. These sessions are an opportunity to ask questions, learn from others with similar experiences, and access up-to-date resources to help you take charge of your health.

Hepatitis B Vaccination Clinics

People with CKD are more vulnerable to infections, which is why vaccination is an essential part of your kidney care. We coordinate and facilitate Hepatitis B vaccination clinics; potentially needing dialysis in the future means you might be more exposed to this virus. Protecting your health with this vaccination is a simple but powerful step in managing your overall well-being.

Low Clearance Clinic for Advanced Kidney Disease

For patients with advanced kidney disease, we coordinate the Low Clearance Clinic, designed to provide specialised care as kidney function declines. This clinic focuses on managing symptoms, discussing treatment options like dialysis or transplant, and preparing you for the transition to more intensive kidney care. We work alongside the kidney doctors in this, and our role is to ensure you receive holistic care and support, tailored to your specific needs.

Coordinating ‘Patients Know Best’

We work closely with the digital platform ‘Patients Know Best’ to ensure you have easy access to your medical information and treatment plan. This tool allows you to communicate with your healthcare team, track your progress, and stay on top of appointments, tests, and treatments. It’s an excellent way to stay connected and involved in your care, providing a comprehensive view of your health.
